I am not sure of the exact model, but I included a picture of the manufacturer's engraving on the receiver.
Barrel is 24 inches long. Near the breech, there is an engraving that says "12 gauge choke" but it does not specify what type of choke it is.
The forend is a bit loose, but it does not affect the function of the gun.
